Instructions

For all clients taking classes please upload

1. Upload Drivers License or State Photo Id
2. Upload your evaluation or screening (if have had one)
3. Clients with Zoom Classes Must fill out the forms at this link prior to classes starting
4. Upload Special State forms (if you need them filled out.)
5. Complete a release form for the Authority requesting you to take this class. This is usually the Court, Probation, your Attorney or Employer. Fill out a Release from for each party you need your Certificate sent to with all the details.  If you need a proof of enrollment, you can download this from the designated area on this page.
6. Your Certificate of Completion can be downloaded from the right side of this page once your class has been marked as completed.
